MPJE: Know your DEA Forms

Pharmacists should be aware of the following DEA forms in their handling of controlled substances:

  • DEA Form 224: Application for Registration Under the Controlled Substances Act. This is the form pharmacies use to apply for a new registration with the DEA to dispense controlled substances. It’s a crucial step for any pharmacy before beginning operations involving controlled substances.

  • DEA Form 222: Order Forms for Schedule I and II Controlled Substances. This form is used by registrants to order Schedule I and II controlled substances.

  • DEA Form 106: Report of Theft or Loss of Controlled Substances. Pharmacists use this form to report any theft or significant loss of controlled substances. Timely reporting of such incidents is critical for compliance and to assist in investigations.

  • DEA Form 41: Registrants Inventory of Drugs Surrendered. This form is used to document the destruction of controlled substances. It is essential for maintaining accurate records of inventory and ensuring that unusable or expired drugs are disposed of properly.
